Therapy: Practice & Billing Policies

Contact Policies
Your primary contact person for therapy services is Sarahbean Falo, LMHC. Sarahbean is available from 10am to 4pm EST, Monday through Friday. Sarahbean communicates by email (hello@sarahbean.co) and scheduled Zoom or telephone call only. Please do not text, Facebook message, Instagram DM, or use other means to communicate for non-urgent matters. For active clients: These policies are different: please refer to your contract.
Email Policies
Please allow 2 business days for a response by email, although Sarahbean strives to respond much sooner.
Time Off Policy
Sarahbean therapy services are closed on weekends and federal and state holidays. From time to time, you will be notified of additional days off.
Appointments and Cancellations
Please remember to cancel or reschedule 24 hours in advance. You will be responsible for the entire cancellation fee ($85) if cancellation is less than 24 hours in advance. Insurance will not cover the cancellation fee.
The standard meeting time for psychotherapy is 55 minutes. It is up to you, however, to determine the length of time of your sessions. Requests to change to a shorter session (45, or 30 minutes) needs to be discussed with the therapist in order for time to be scheduled in advance.
Cancellations will be subject to a $85 charge if NOT RECEIVED AT LEAST 24 HOURS IN ADVANCE. This is necessary because a time commitment is made to you and is held exclusively for you. If you are late for a session, you may lose some of that session time. If you can reschedule the session within the same week, the cancellation fee will be waived. If you or your child have symptoms of an illness that impede functioning on telehealth, please give as much notice as you can.
Session Fees
The initial Intake Session lasts about an hour and costs $200/session. Other counseling sessions may be 30, 45, or 60 minutes, depending on your needs.
Treatment fees are as follows:
60-minute session: $200
45-minute session: $175
30-minute session: $125
Initial Intake session: $225
Collateral Contact or written reports: This includes conversations with school personnel, other providers, or reports written upon your request (always with your expressed written permission).
First 15 minutes: Free
Up to 30 minutes: $65
Payments
Payment Options: We accept Credit Card payments through Stripe payment processing, Headway (for insurance billing), and gethealthie.com, which are connected to your intake process once we have established therapeutic services.
General Policies:
You are responsible for keeping all credit card details and contact information current to keep your account in good standing. Please email hello@sarahbean.co to update your billing information, or log into your private portal to update your payment method.
For private pay clients, I take all payments via any major credit card, HSA or FSA through the client portal (via Stripe), and your card on file will be charged within 12 hours after your session through a feature called "Autopay." If this arrangement doesn't work for you, please contact me and we will find a solution.
Good Faith Estimate
Under the law, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. Learn more here about the No Surprises Act: https://www.cms.gov/nosurprises
Insurance
I accept most BCBS plans and Point32 Health plans through Headway. Please note: in order to use your insurance benefits, you—or your child—must have the symptomology of a diagnosis consistent with the DSM-5. We can discuss this further in our intake session.
Once we establish our working relationship, you will receive these contracts via client portal, and if using insurance, you will enter your Insurance and Billing information into the Headway portal. This can be initially confusing, but once we have it set up, have run an eligibility check, and clarified your co-pay or deductible, it can run automatically.
If you want to pay with insurance AND an HSA/FSA card, you will have to manually enter the payment after every session in the portal. If you choose to pay by credit card, you can enroll seamlessly into Autopay and never miss a payment.
If you choose to pay out of pocket (private pay), or have other insurance with Out-of-Network coverage (usually a PPO plan), your insurance provider may reimburse a percentage of the counseling fee directly to you. The full fee is required, from you, at the time of service. Please call your insurance provider to ensure you have out-of-network coverage for mental health counseling. I can provide you with a monthly statement (a "Superbill") for you to submit to your insurance for reimbursement. Please note that if you want to go the Superbill route, you must have a DSM-5 diagnosis on file.
It is your responsibility to check with your insurance provider concerning coverage and payment.
Billing & Delinquency
You will be billed automatically every 30 days (or the nearest weekday) for recurring charges, or at intervals otherwise agreed in our contract.
Delinquency Policies: If the credit card is declined, we will continue to attempt to charge the card on file every day for 3 days. We will contact you for updated contact and payment information.
Delinquent Accounts: After 30 days without payment, your account will be considered delinquent. All accounts 30 days past due may be disabled or deactivated until balances are paid in full.
Billing Disputes
In the unlikely event of a disagreement between us regarding your services or deliverables, you agree to make a good faith attempt for 30 days to communicate with Sarahbean prior to initiating a chargeback.
Collection Fees
Returned Item Fee: There is a $50.00 insufficient funds check fee for returned checks in addition to the amount still due.
Late Fee: Invoices which remain unpaid after 30 days may be assessed a late fee of $50.00 per month.
Chargebacks: If a customer initiates a chargeback, we will assess a $250.00 processing fee for each individual chargeback.
Collections Fees: In the event an account is submitted to a third-party collections service, the client will be responsible for the full amount of the collections fees, in addition to any other fees previously assessed on the account.
Cancellation
Cancellation of an account does not dismiss outstanding invoices or the contracted commitments. At the time of cancellation, any outstanding balance must be settled. All canceled accounts with an outstanding balance may be automatically turned over to a third-party collection service. This paragraph does not invalidate our refund policy.
Chargebacks
Regardless of the outcome of a chargeback dispute by a credit card company or payment processor, we reserve the legal right to enforce our agreements and collect past due balances in court.
Telephone Accessibility
If you need to contact me between sessions, please email me, text me or leave a message on my voice mail. I am often not immediately available; however, I will attempt to return your call within 48 hours. In the event that you are out of town, sick or need additional support, phone sessions are available. If a true emergency situation arises, please call 911 or any local emergency room.
Social Media and Telecommunication
Due to the importance of your confidentiality and the importance of minimizing dual relationships, I do not accept friend or contact requests from current or former clients on any social networking site (Facebook, LinkedIn, etc). I believe that adding clients as friends or contacts on these sites can compromise your confidentiality and our respective privacy. It may also blur the boundaries of our therapeutic relationship. If you have questions about this, please bring them up when we meet and we can talk more about it.
Electronic Communication
I use a HIPAA-compliant Google Workspace platform for my email service and HIPAA-compliant phone service through phone.com. My preferred methods of communication in-between sessions are text and email. However, I cannot ensure the confidentiality of any form of communication through electronic media. If you prefer to communicate via email or text messaging for issues regarding scheduling or cancellations, I will do so. While I may try to return messages in a timely manner, I cannot guarantee immediate response and request that you do not use these methods of communication to discuss therapeutic content and/or request assistance for emergencies.
Telehealth Services
Services by electronic means, including but not limited to telephone communication, the Internet, facsimile machines, and e-mail is considered telehealth by the Commonwealth of Massachusetts. Under Massachusetts telehealth laws and regulations, telehealth is broadly defined as the use of information technology to deliver medical and mental health services and information from one location to another. If you and your therapist choose to use information technology for some or all of your treatment, you need to understand that:
(1) You retain the option to withhold or withdraw consent at any time without affecting the right to future care or treatment or risking the loss or withdrawal of any program benefits to which you would otherwise be entitled.
(2) All existing confidentiality protections under HIPAA and Massachusetts law are equally applicable.
(3) Your access to all medical information transmitted during a telehealth consultation is guaranteed, and copies of this information are available for a reasonable fee.
(4) Dissemination of any of your identifiable images or information from the telehealth interaction to researchers or other entities shall not occur without your consent.
(5) There are potential risks, consequences, and benefits of telehealth. Potential benefits include, but are not limited to improved communication capabilities, providing convenient access to up-to-date information, consultations, support, reduced costs, improved quality, change in the conditions of practice, improved access to therapy, better continuity of care, and reduction of lost work time and travel costs. Effective therapy is often facilitated when the therapist gathers within a session or a series of sessions, a multitude of observations, information, and experiences about the client. Therapists may make clinical assessments, diagnosis, and interventions based not only on direct verbal or auditory communications, written reports, and third person consultations, but also from direct visual and olfactory observations, information, and experiences. When using information technology in therapy services, potential risks include, but are not limited to the therapist's inability to make visual and olfactory observations of clinically or therapeutically potentially relevant issues such as: your physical condition including deformities, apparent height and weight, body type, attractiveness relative to social and cultural norms or standards, gait and motor coordination, posture, work speed, any noteworthy mannerism or gestures, physical or medical conditions including bruises or injuries, basic grooming and hygiene including appropriateness of dress, eye contact (including any changes in the previously listed issues), sex, chronological and apparent age, ethnicity, facial and body language, and congruence of language and facial or bodily expression. Potential consequences thus include the therapist not being aware of what he or she would consider important information, that you may not recognize as significant to present verbally to the therapist.
Children (Humans Under Age 18)
If you are a minor, your parents may be legally entitled to some information about your therapy. I will discuss with you and your parents what information is appropriate for them to receive and which issues are more appropriately kept confidential.
Graduation
Ending relationships can be difficult. Therefore, it is important to have a graduation process in order to achieve some closure. The appropriate length of the graduation depends on the length and intensity of the treatment. I may terminate treatment after appropriate discussion with you and a graduation process if I determine that the psychotherapy is not being effectively used or if you are in default on payment. I will not terminate the therapeutic relationship without first discussing and exploring the reasons and purpose of graduating. If therapy is terminated for any reason or you request another therapist, I will provide you with a list of qualified psychotherapists to treat you. You may also choose someone on your own or from another referral source.
Should you fail to schedule an appointment for three consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, I must consider the professional relationship discontinued.
